Sidhmam – Psoriasis

 

Overview

Immune response of a human body to the uncertain factors leads to the accelerated inflammatory proliferation of the ailing cells of the skin known as Psoriasis. Psoriasis is a chronic immune-mediated inflammatory condition mainly affecting the skin and joint. In Ayurveda, many herbs have proven efficacy in psoriasis, but the multifaceted etiology of the disease needs a multimodal treatment approach.

The chronic nature, recurring pattern, and visibility of psoriasis produce a great adverse impact on the psychological and social aspects of patients  life. Psychosocial disability affects their daily work as well as social interactions. In Ayurveda, skin diseases have collectively considered under a common term of Kushtha. Most of the formulations described in Kushtha Chikitsa have efficiently been used by the physicians to treat different skin ailments.

Symptoms for Psoriasis

Symptoms of Psoriasis differ from person to person which depends on the type of Psoriasis.

The most common symptoms of Psoriasis include:

  • Inflamed patches of skin
  • Plaques on the red patches
  • Dry skin that may crack and bleed
  • Soreness around patches
  • Itching/burning sensations around patches
  • Thick/pitted nails
  • Swollen joints
  • Stress – Unusual high stress may trigger a flare-up.
  • Heavy alcohol also triggers flare-up.

Types

There are various types of psoriasis that have different characteristics. According to the International Psoriasis Council (IPC), there are 5 forms in total.

  • Chronic psoriasis plaque
  • Psoriasis Guttate
  • Psoriasis Pustular
  • Psoriasis Erythrodermic
  • Ekakusta, Kitibha and Sidma are some of the types of Kshudra and Maha Kustas (skin diseases) narrated in Ayurveda that closely resemble the classical symptoms of Psoriasis.

Ayurvedic Treatment for Psoriasis

Psoriasis is believed to occur due to reduction in all three doshas (Vata, Pitta, Kapha) in varying degree, but mainly Vata and Kapha according to the Ayurveda. Ayurvedic treatment for psoriasis involves a holistic approach that addresses the root cause of the disease by restoring balance to the body and mind.

The vata mainly causes pain, dryness and scaling of the skin. Pitha vitiation leads to a burning feeling, redness, inflammation, etc. and that of Kapha causes rashes, itching, discharge, thickening of skin, etc. From these symptoms, the involvement of Dosha is analyzed carefully in each patient, which is most important in the Ayurvedic treatment for psoriasis or the management of any disease.

Specific Ayurvedic treatment

Shodhana chikitsa (Detoxification) can be done through Panchakarma therapies like Vamana, Virechana, Rakta mokshna etc., for chronic cases of 5 plus years of psoriasis  repeated detox may be advised

Shamana chikitsa through proper management of internal medication and external applications

Rasayana chikitsa (immunomodulation) to arrest recurring and boost immunity

Changes in diet and lifestyle

The irregular lifestyle, consumption of foodstuffs such as dairy products with fish, yogurt, paneer, black gram, seafood sour and salted items that are advised not to be eaten together which activates the pathogenesis.

The consumption of alcohol and tobacco will act as a catalyst here.

Ayurveda also looks on to the effect of stress  in the pathogenesis of Psoriasis.

Vicharchika (Eczema)

 Overview

In Ayurveda all skin disorders have been classified mainly into two groups , Mahakushtas & Kshudra Kushtas. Vicharchika (Eczema) is one among the Kshudra Kushta (Minor skin disorder) mentiond inayurveda. Vicharchika is characterized by skin manifestation having the symptoms Kandu (Itching sensation), Pidika (Papule) Shyava Varna (Blackish brown discoloration) and Bahusrava (Excessive exudation). It is correlated with eczema in modern dermatology, based on the clinical presentations.

The term Eczema means to” Boil out” ( ec = out, zema = boil) that the skin is boiling out or oozing out. Eczema is an inflammatory reaction pattern of the skin. Characterized by variable intensity of itching and soreness, crusting, dryness, erythema, excoriation, exudation, fissuring, hyper pigmentation, lichenification, oozing, scaling & vasculation. The most common type of eczema is known as atopic dermatitis or atopic eczema. No satisfactory treatment is available for eczema in modern medicine, some antihistamines & topical steroids are used for treatment of eczema. According to Ayurveda, Vicharika can be classified into various types based on the doshas involved:

 Types & Symptoms

Vataja Vicharika: This type of eczema is primarily associated with imbalanced Vata dosha. The skin becomes dry, rough, and scaly, with symptoms aggravated by cold and dry weather.

Pittaja Vicharika: Pitta dosha imbalance leads to redness, inflammation, and intense itching in the affected areas. Exposure to heat, spicy food, and excessive sun can worsen the symptoms.

Kaphaja Vicharika: Kapha dosha dominance causes thick, oozing lesions with excessive itching and a feeling of heaviness. Cold and damp weather conditions tend to exacerbate the symptoms.

Specific Ayurvedic treatment.

Treatment principles for Vicharchika – treatment can be divided in Sroto Sodhana by Panchakarma therapies & Dosha Shamana by internal medications. Sodhana may involve Vamana, Virechana & Rakta Mokshana according to the condition & in Shaman treatment medicines with Pitta Kaphahara properties in acute stage are helpful. For chronic stage of Vicharchika Vata Kaphahara medicines are useful.

Topical applications of Ayurvedic medicated oils, creams, and pastes, along with medicated baths, help soothe the affected skin, reduce inflammation, and improve skin texture.

 Visarpa (Pemphigus)

 Overview

Twacha word in Ayurveda is derived from the word ‘Tvac’ which means ‘to cover something’. It is first line of defence which protects body in various ways. According to Ayurvedic classics, Twacha Rogas are produced by the imbalance of Tridoshas-Dhatus and Raktadushti. In Ayurveda all the skin disorders are collectively named under the heading of Kushta. Ayurveda  Acharyas have described all the Kushta are having the involvement of all the three Dosha’s. But the signs, symptoms and the treatment depend on the predominance of the Dosha involved.

“Visarpa” is one of the most common skin disorder which explained in Ayurveda. The Visarpa which is an active transmitting viral disease causing painful skin rash with acute onset blisters in localised areas. Visarpa predominantly involves Pitta Dosha.

Visarpa can be compared to Pemphigus and  is a chronic autoimmune disorder characterized by blistering and ulceration of the skin and mucous membranes.

Types &Symptoms

Vatika Visarpa :  is the result of obstruction caused by aggravated Doshas. It shows resemblance to the acute skin infections such as Erysipelas. Its clinical features are burning sensation, fever, pain, affected area becomes edematous and red, small blackish or reddish blisters with thin clear reddish and scanty discharge are seen.

 PittaVisarpa: causes various problems such as fever, pain, burning sensation etc. Pustules are formed in the affected part causing intense pain and burning sensation. It can be co-related with the skin diseases such as Erysipelas, Herpes and burns etc.

Kaphaja Visarpa: spreads slowly in the body and it shows resemblance to the Erysipelas. It causes fever, vomiting, chills etc. The affected area becomes edematous, red with pale coloured eruptions covered by thick skin. They can leave deep seated ulcers also if not treated on time.

Agni Visarpa: is a very serious condition resembling septicemia. Affected area gets covered by red coloured blisters. Person suffers from severe burning sensation and becomes restless. It spread rapidly to the vital organs and it is considered as incurable.

Kardam Visarpa : is caused due to Pitta and Kapha aggravating factors. The affected area looks like mud and hence the name Kardam is given. It is a serious skin ailment of necrotizing nature and is considered as similar to necrotizing fascilitis and gangrene etc. Affected area gets covered by reddish, pale eruptions which are suppurated, having putrid flesh and skin. When they are touched they burst and throw out decomposed and putrefied flesh showing blood vessels, ligaments and cadaverous smell. It is mentioned as incurable Visarpa in various Ayurvedic texts.

Granthi Visarpa: is caused by aggravated Rakta and Kapha-Pitta Dosha. It causes glandular enlargements which are red in colour, small and painful located in the bones, muscles, blood vessels etc. It can be correlated with B-cell lymphoma and is considered as incurable.

Ayurvedic Treatment for  Visarpa

The Ayurvedic approach to treating Visarpa involves a combination of internal medications, external applications, and lifestyle modifications. Internal medications, consisting of herbal formulations and natural compounds, are prescribed to strengthen the immune system, detoxify the body, and control the autoimmune response. Visphota Kushta (folliculitis)

Overview

Visphota Kushta (blistering skin disease) is characterized by transparent blisters with thin skin covering. It can be considered under the broad spectrum of vesiculobullous diseases which are characterized by vesicles and bullae. Visphotam, can be also called as folliculitis,  a skin condition that occurs when hair follicles become inflamed due to bacterial or fungal infections, irritation, or blockage.

The main causative factors in the manifestation of the pathology of Visphota Kushta are Pitta and Kapha Dosha associated with Dhatu’s like Rasa and Rakta.

 Symptoms

  • Rash with small bumps
  • Tenderness or pain
  • Itching
  • Pustules
  • Burning sensation
  • Thick crusts

Ayurvedic Treatment for Visphota

Based on the severity of folliculitis and the doshas imbalances , the Ayurvedic practitioner will recommend Samana drugs. Ayurvedic therapies such as Panchakarma (detoxification treatments) and rejuvenation therapies may be recommended to support the healing process and restore overall health.

  • Abhyanga with medicated oils like chemparathyadi kera taila ,malathyadi kera taila etc
  • Kashaya Dhara
  • Takra Dhara
  • Lepana with choornas of drugs like nimba , chandana etc

Changes in diet and lifestyle

A proper balanced diet with essential nutrients based on Dosha levels is crucial to maintain the bodies Dosha equilibrium.

Keep the body hydrated by consuming at least 2-3 L of water in a day and maintain proper hygiene.

Try to avoid Pitta aggravating foods like fried food ,food with excess salt , spice , and sourness. Avoid consuming stale food and overeating.

Kitibham ( lichen planus)

 Overview

All the skin diseases in Ayurveda  have been discussed under the broad heading of Kushta which is further divided into Mahakusta and Kshudra Kusta. Kitibha Kusta is included under Kshudra Kusta which simulates with signs and symptoms Lichen Planus. striae. According to Ayurveda, Kitibham (lichen planus) is a disorder, primarily associated with an imbalance of the “Pitta” and “Vata” Doshas.

Symptoms

It is a chronic inflammatory skin disorder. It manifests as itchy, flat-topped, reddish-purple bumps on the skin that may exhibit a distinctive lacy white pattern called Wickham’s striae.

Ayurvedic Treatment for Kitibham

Detoxification and Purification (Panchakarma): Panchakarma, a cornerstone of Ayurvedic therapy, involves the systematic removal of toxins from the body. The treatment includes procedures like Virechana (therapeutic purgation) and Vamana (therapeutic emesis) to eliminate toxins and restore the balance of doshas. These processes not only help in the detoxification of the body but also rejuvenate the immune system.

Darunakam- Dandruff

 Overview

Unhealthy hair is always a reason of concern not just for females but also for males. Damaged hair and scalp are always frustrating factor for youth. Dandruff is a common chronic scalp disorder that is characterized by flaking of the skin of the scalp. Dandruff can be correlated to Darunakam  in Ayurveda. Darunakam is a mentioned it under Kshudra Rogas or Kapalgat Rogas.

Symptoms

Kandu (Itching), Kesha  Chyuti (Hair  fall), Swapa (Abnormality of touch sensation), Rukshata (Dryness and roughness of scalp), Twak Sphutana (is cracking  or  splitting  of scalp).

The   treatment   principles   mentioned   are  Siravyadha  (Raktamokshana),  Shiro  abhyanga,   Nasya,  Shirolepana,    Shiro Prakshalana and Shirobasti

  • Additionally, stress management techniques such as yoga, meditation, and regular exercise are recommended to promote overall well-being and support the healing process.
